Behavioral Health Friends and Family Input Form

The FFIF is available as a fillable and downloadable PDF, allowing you to easily get critical information about your loved one to their behavioral health providers.

About the Friends and Family Input Form (FFIF)
The Friends and Family Input Form (FFIF) is a tool that the Behavioral Health Administration developed to allow loved ones of a person receiving mental health or substance use services in Colorado to legally give the treating provider information about their loved one that may be helpful in their treatment.
BHA developed the FFIF pursuant to House Bill 24-1217, traveling to six different locations throughout Colorado to engage with parents, friends, and loved ones of persons who have received mental health and substance use care throughout our regional networks. The BHA, fellow state agencies, behavioral health providers, and legislators alike all actively listened to the need for this form, and how it could best be utilized in our evolving behavioral health networks throughout the state, resulting in the first version of the form becoming available for use in July of 2025.
Who can use the FFIF
Anyone who cares about the well-being of someone receiving mental health and substance use services can fill out the form. People completing the form must do their best to make sure the information they share is true and accurate.
Providers that accept the FFFIF
Any provider, of any mental health or substance use service, may accept the form. Providers are not required to accept the form. There is no penalty associated with a provider of behavioral health services acting, or not acting, on any information provided to them through the form.
How the FFIF is shared
The FFIF gives friends and family members a way to share clear and helpful information with a provider. Providers may choose to include the form in a patient’s medical record, but they don’t have to. Providers can also decide whether or not to tell their patients that they received an FFIF, even if the person who submitted it asked for confidentiality.
